About the Role:

The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence. This position requires conducting thorough patient assessments to develop and implement individualized treatment plans that address specific physical impairments and functional limitations. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure continuity of care and optimize therapeutic outcomes. By providing education on exercises, safety, and adaptive techniques, the therapist empowers patients to manage their conditions effectively and prevent further injury. Ultimately, this role contributes significantly to enhancing patients' quality of life by facilitating their return to daily activities in a safe and supportive environment.

Minimum Qualifications:

  • Valid and unrestricted Physical Therapist license in the state(s) of practice within the United States.
  • Graduation from an accredited Physical Therapy program with a degree in Physical Therapy (DPT preferred).
  • Current CPR/BLS certification.
  • Proven experience or clinical training in physical therapy, preferably including home health or outpatient settings.
  • Strong knowledge of anatomy, physiology, and therapeutic techniques relevant to physical rehabilitation.

Preferred Qualifications:

  • Experience working in home health care or community-based rehabilitation.
  • Certification in specialized therapy techniques such as wound care, neurological rehabilitation, or geriatrics.
  • Familiarity with electronic medical records (EMR) systems used in home health settings.
  • Excellent interpersonal and communication skills to engage effectively with diverse patient populations.
  • Ability to work independently with strong organizational and time management skills.

Responsibilities:

  • Perform comprehensive evaluations of patients' physical abilities, limitations, and needs in a home setting.
  • Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings.
  • Provide hands-on therapy, including therapeutic exercises, manual therapy, and functional training to improve strength, balance, and mobility.
  • Educate patients and caregivers on home exercise programs, safety precautions, and use of assistive devices.
  • Document patient progress accurately and communicate effectively with physicians, nurses, and other healthcare professionals to coordinate care.
  • Manage scheduling and travel efficiently to meet patient appointment needs across various locations.
  • Stay current with best practices, clinical guidelines, and regulatory requirements related to home health physical therapy.
